This fixes some links to grammar production names. Some of these links were using the wrong name. Also, the explicit link definitions aren't necessary anymore.

These links were using the wrong name (_LazyBooleanOperatorExpression_)
which doesn't exist.
This was missing an `s` as a plural.
Now that mdbook-spec auto-generates link definitions for grammar names,
there's no need to be explicit about these.

In rust-lang#1787 I missed linkify-ing
references to grammar rules that weren't links. This makes sure that
they are linked and validated.
